2021 Open Enrollment for Health Benefits

**IMPORTANT INFORMATION** 2021 DEPENDENT VERIFICATION REVIEW (DVR)
The District is completing a Dependent Verification Review (DVR) during open enrollment. This will require that employees with dependents enrolled on any of their health insurance plans must provide proof of eligibility (birth certificate for dependent children and tax documents for spouses or registered domestic partners).
 
American Fidelity representatives will be meeting with staff to verify their dependent coverage. They will be conducting virtual meetings beginning on Tuesday, July 27th through Wednesday, August 11th, and will be at the District Office for in-person meetings on Friday, August 6th and Monday, August 9th.

2021 OPEN ENROLLMENT PERIOD - JULY 26TH THROUGH AUGUST 11TH
Benefit eligible employees may enroll in health benefits by accessing the BenefitFocus portal here. All benefit changes will become effective October 1st. All benefit eligible employees must complete the Authorization for Payroll Deduction form.
 
**If you are waiving benefits, you will also need to submit the Health Benefits Waiver form and proof of other medical coverage.

2021 KAISER PERMANENTE HMO PLANS
With the Kaiser Permanente Health Maintenance Organization (HMO) plans, services must be obtained at a Kaiser facility, except in the case of emergency. Kaiser integrates all elements of healthcare such as physicians, medical centers, pharmacy and administration in one convenience facility. In addition, Kaiser offers online tools, including a mobile app, so you can e-mail your doctor's office, make appointments, refill prescriptions and more. You can also have phone and video appointments for non-urgent health issues at no charge. Kaiser HMO plan options include:
2021 BLUE SHIELD OF CALIFORNIA HMO PLANS
With the Blue Shield of California Health Maintenance Organization (HMO) plans, you are required to select a Primary Care Physician (PCP) within the Blue Shield HMO network. Your PCP will coordinate all of your medical care. You will receive benefits only if you use the doctors, clinics and hospitals that belong to the medical group in which you are enrolled, except in the case of an emergency. Any specialty care you need will be coordinated through your PCP and will generally require a referral for authorization. Blue Shield offers online tools, including a mobile app, so you can make appointments, refill prescriptions and more. You also have access to Teladoc for non-urgent phone and video appointments for a $5 copay. Blue Shield of California HMO plan options include:
2021 BLUE SHIELD OF CALIFORNIA PPO PLANS
The Blue Shield of California Preferred Provider Organization (PPO) plans allow you to direct your own care. When you receive care from a physician who is a member of the Blue Shield PPO network, a greater percentage of the entire cost will be paid by the insurance plan. You may also obtain services using a non-network provider; however, you will be responsible for the difference between the covered amount and the actual charges, and you may be responsible for filing claims. 2021 DELTA DENTAL PPO INCENTIVE PLAN
The Delta Dental PPO plan is a preferred provider dental plan. The benefits cover a wide range of dental services. You may visit a PPO dentist and benefit from the negotiated rate or visit a non-PPO dentist. When you utilize a PPO dentist, your out-of-pocket expenses will be less. You may also obtain a non-PPO dentist; however, you will be responsible for the difference between the covered amount and the actual charges and you may be responsible for filing claims. 
 
With the Delta Dental PPO Incentive Dental Plan, Delta Dental pays 70% of the allowed fees for covered diagnostic, preventive basic, and major services during the first year you are eligible. This percentage increases 10% each year (to a maximum of 100%) for each family member, provided that person visits the dentist at least once during the year. If a family member does not use the plan during a calendar year, the percentage remains at the level reached the previous year. If a family member becomes ineligible for benefits and later regains eligibility, the percentage will drop back to 70%.
2021 VISION SERVICE PLAN (VSP)
The VSP vision plan provides professional vision care and high quality lenses and frames through a broad network of optical specialists. You will receive richer benefits if you utilize a network provider. If you utilize a non-network provider, you will be responsible to pay all charges at the time of your appointment and will be required to file an itemized claim with VSP Vision.
Voya Basic Life and AD&D Insurance
Life insurance protects your family or other beneficiaries in the event of your death while you are still actively employed with the Centralia School District. The District pays for coverage, offered through Voya Life Insurance, in the amount of $25,000. The amount reduces by 35% at age 70, and by 50% at age 75. If your death is due to a covered accident or injury, your beneficiary will receive an additional amount through Accidental Death and Dismemberment (AD&D) coverage.
Voya Supplemental Life Insurance
In addition to the company provided Basic Life and AD&D benefits, you may elect to purchase additional Term Life insurance at discounted group rates provided by Voya. You pay for this coverage with after-tax dollars through convenient payroll deductions.
